The Signal Before the Storm: Passive Monitoring and the Mission to Reach Veterans Earlier

Every day, 17.5 veterans die by suicide. The VA's 2025 National Veteran Suicide Prevention Annual Report confirmed that 61 percent of those who died in 2023 were not receiving VA health care in the final year of their lives. The crisis is not a lack of care. The crisis is that care arrives too late.

Passive physiological signal collection is the precursor to changing that timeline. Wrist-worn devices, chest-band monitors, smart rings, adhesive biosensor patches, and embedded textile sensors all capture continuous biometric data: heart rate variability (HRV), blood oxygen saturation, skin temperature, respiratory rate, and cardiac rhythm. The specific device is immaterial. What matters is the unbroken stream of physiological data these devices generate without requiring the wearer to do anything. The body broadcasts distress through suppressed HRV, irregular cardiac rhythm, and disrupted sleep architecture days or weeks before that distress becomes visible to peers, family, or clinicians. A veteran compounding chronic pain, isolation, and sleep disruption will register declining biometric trends long before articulating a need for help. Passive signal collection captures that broadcast. Without collection, the signal dissipates unrecorded.

Collection alone, however, saves no one. Raw biometric data sitting in a device app is just numbers on a screen. The signal requires interpretation, direction, and connection to human support. This is where Mission URSA operates.

Mission URSA, a 501(c)(3) nonprofit and public initiative of Ursa for Veterans, builds the framework that turns collected signals into life-saving outcomes. Named for Ursa Minor and its North Star, the organization centers on a conviction that support should begin before crisis. Their "Reveal, Guide, Orient" model defines a sequential response to the physiological evidence that passive monitoring provides. "Reveal" surfaces the importance of the signal, translating raw biometric trends into meaningful awareness that something is shifting. "Orient" points the veteran toward the proper direction, identifying the right resource, the right conversation, or the right intervention for their specific situation. "Guide" walks them toward help through peer engagement, AI-assisted wellness systems, and organizational support networks.

The relationship between signal collection and Mission URSA's framework is sequential and interdependent. Signal collection is the foundational input. URSA's model is the response architecture. One produces evidence of emerging distress. The other ensures that evidence reaches someone who can act on the evidence. Stress, isolation, and disengagement surface long before a visible breaking point. Mission URSA's research and AI-assisted systems give organizations the visibility they currently lack to recognize those earlier signals and intervene with purpose.

The veteran who most needs help is often the one least likely to ask for help. Passive signal collection produces the earliest measurable proof that something is wrong. Mission URSA builds the system that reveals what the signal means, orients the veteran toward the right path, and guides them to support. Neither saves a life alone. Together, they close the gap between silent suffering and meaningful intervention. The North Star works best for travelers who cannot yet see the path ahead.
